a. DTI Certificate
b. SSS Clearance
c. Barangay Business Clearance
Old Clients, submit additional requirements such as:
a. Previous business permit
b. Sworn statement of gross sales for preceding year

Submit all requirements to Licensing Section (Treasury Office) and fill out an application form.

Assess the business of the client for payment.

Inspect the Official Receipt and sign the application if in order.

Proceed to Rural Health Unit for health and sanitary inspection.

The sanitary inspector will check sanitary status of business.

Proceed to Municipal Planning and Development Coordinator’s Office for Zoning Permit.

The MPDC will check and approve whether the business is in accordance with zonification plan.

Proceed to Engineering Department.

The Municipal Engineer will perform building inspection.

Proceed to Fire Department.

The Fire Marshal will perform fire protection safety and status of the building.

Proceed to Mayor’s Office.

Approve the application if found complete.
