b. SSS Clearance
c. Barangay Business Clearance
Old Clients, submit additional requirements such as:
a. Previous business permit
b. Sworn statement of gross sales for preceding year

Submit all requirements to Licensing Section (Treasury Office) and fill out an application form. |
Assess the business of the client for payment. |

Proceed to Rural Health Unit for health and sanitary inspection. |
The sanitary inspector will check sanitary status of business. |

Proceed to Municipal Planning and Development Coordinator’s Office for Zoning Permit. |
The MPDC will check and approve whether the business is in accordance with zonification plan. |

Proceed to Engineering Department. |
The Municipal Engineer will perform building inspection. |

Proceed to Fire Department. |
The Fire Marshal will perform fire protection safety and status of the building. |

Proceed to Mayor’s Office. |
Approve the application if found complete. |
